EU Digital Certificates
EU Digital certificates are being issued by the HSE. We do not have any input into how or when they are sent. We have provided vaccination information to the HSE for all patients we have vaccinated, however we do not submit addresses or email addresses. These are obtained from information already held by the HSE or The Dept. of Social Protection.

COVID Vaccine Rollout Information
We have now completed our COVID-19 vaccine rollout.
It is now possible for everyone over the age of 12 to register for a vaccine and also walk-in vaccination centres are opening around the country.
To get information regarding the portal registration please contact the HSE helpline on 1850 24 1850. We do not have any information in relation to this part of the vaccine program.

All people over the age of 12 can now register for a vaccine online by clicking the button below or see the HSE website for the locations and opening hours of new walk in vaccination clinics.
Already Vaccinated Elsewhere?
If you have received your COVID vaccine elsewhere please complete the following form so that we can update our records.
If you already received your Covid Vaccine through hospital or mass vaccination clinic, you can upload the details using the form below please. We will then record these details on your medical file.
You will find the date, batch number and type of vaccine received recorded on the Vaccination Record Card you were given. You can submit your doses separately or together if you have already received both doses.
